We do not store credit card details nor do we share financial details with any 3rd parties.
Due to the nature of our services, it is necessary to collect certain personal information from Clients. All information collected is treated as STRICTLY CONFIDENTIAL, and Model Health will not share or redistribute your information with any third party except as necessary to provide services purchased by the Client, or as required by law.
Any information gathered from a Client is simply for our records and, if applicable, necessary to provide the services to the Client for which we have been contracted. All Terms and Conditions are subject to change.
FITNESS UNIT CONTRACTS & COMPANY POLICIES
In addition to signing a PARQ and Terms and Conditions form, the client will be required to sign and return the following forms to Model Health prior to receiving Training Program Design, One to One Training or semi-private training or beginning any Group Training program:
- Physical Activity Readiness (Health & Medical) Questionnaire (PAR-Q)
- Doctors’ Consent Form(if needed).
- Waiver, Release, and Assumption of Risk Form
- Commitment Agreement and understanding of Model Health’s ‘core values’.
If you have any of the following physical conditions, you may be required to have a Medical Clearance and Doctors’ Consent Form:
- Hypertension (>145/95 mm Hg)
- Hyperlipidemia (cholesterol>220 mg/dl or a total cholesterol-to-HDL ratio of >5.0)
- Family history of heart disease prior to age 60
- Abnormal resting ECG
Unless other arrangements are made, Personal Training sessions, Small Group Training (semi-private) sessions and Large Group Training sessions (last approximately 45 - 50 minutes. In order to provide the best service to all Clients, Model Health cannot commit to extending any particular session beyond its previously scheduled time.
Rates for Model Health. Services are subject to change. Services prepaid for by Client, which are unused at the time of any rate change, will be honored at the price already paid.
CANCELLATION / REFUND POLICIES
A Client will be deemed a ‘no-show’ when they are 15 minutes late for an appointment. No refunds or credits will be given for ‘no-shows’. Any small or large group training sessions missed by the client cannot be refunded or rescheduled.
All cancellations for 121 Personal Training must be made with a minimum of 24 hours advance notice in order to receive credit for the session. Due to an inability to fill the previously blocked time period, Cancellations with less than
24 hours notice given will not qualify for a refund and the client will be charged at the full price for the session. Cancellations must be made by contacting the Model Health Trainer you are working with to be deemed effective.
If Client receives credit for a missed session, the credit must be used within the same block of the missed session, or it will be waived.
If Model Health needs to cancel a scheduled session, Client will receive credit for any such session missed. Sessions are paid for in advance by electronic funds transfer (EFT). Any session not used in the block cannot be carried over.
